I'd make citizenship something to be earned through mandatory military service, not something to be given by birthright, so that citizenship would also have responsibility attached to it. Or, I'd do what they do in Cuba and enforce a quarantine of HIV-positive people. Not that they're horrible people, or anything, but it's the only way to ever get rid of AIDS, save a cure.
